Pick the repair option if you are sending in equipment to be fixed and the return option if you are sending back items you've purchased
Many stores and restaurants have a distinct number and/or letter assignation that identifies them as member of a larger organization or entity.
Provide the name of the manufacturer and if you know it, the model number
If you are returning equipment to us, we recommend that you include the serial numbers for documentation and safety purposes. Please include the serial numbers of any devices you may be shipping to us if applicable. If you have a device where the serial number is unreadable, please make a note of this.

Please be as detailed as you like in the description of what work you would like performed. We understand that you may have multiple issues. Feel free to include all of them.
